PRIVACY POLICY: CSP Medical respects the privacy of all customers and website visitors. This policy represents our commitment to the protection of all personal information disclosed to CSP Medical.

COLLECTION & USE OF PERSONAL INFORMATION:

CONTACT INFORMATION: When first contacting CSP Medical, you may be asked for basic contact information, such as name, company name, address, phone number, e-mail address or similar contact information, for the purpose of providing service and information. Occasional mailings and e-mail newsletters are available upon request. Customers can easily opt in or out of such mailings by request.

WEBSTORE: Our webstore, accessed through www.cspmedical.com can be browsed without providing any identifying information.

CREDIT INFORMATION: Credit card information, whether online or phoned-in, is not stored on paper. The electronic data in the transaction and/or customer record is encrypted and cannot be viewed without secured authorization. To purchase with terms, a credit application is required. This form requires financial information including bank information and credit references.

HUMAN RESOURCES: Applications, resumes, and CV's are generally kept for six months. If the individual is hired by CSP Medical then the documents become part of the employee's file.

DISCLOSURE OF PERSONAL INFORMATION: CSP Medical does not sell or give personal information to third parties, with the possible exception of basic contact information for the purposes of providing or delivering requested goods, services or information.

ACCESS TO INFORMATION: Your information is stored in your account record in a secure database.

What are your Terms and Conditions of Sale ?

WE HEREBY OBJECT TO AND REJECT THE PROVISIONS OF ANY AND ALL PURCHASE ORDERS, ACKNOWLEDGEMENTS OR OTHER DOCUMENTS PROVIDED BY YOU WHICH ARE INCONSISTENT WITH OR OTHERWISE PURPORT TO MODIFY THESE TERMS AND CONDITIONS. THESE TERMS AND CONDITIONS SHALL BE THE COMPLETE AND EXCLUSIVE CONTRACT BETWEEN US GOVERNING THE PURCHASE AND SALE OF THE PRODUCTS. THESE TERMS AND CONDITIONS MAY BE MODIFIED ONLY IN WRITING SIGNED BY OUR AUTHORIZED REPRESENTATIVE AND BY YOU. NO PRIOR PROPOSALS, QUOTATIONS, STATEMENTS, FORECASTS, COURSE OF DEALING OR USAGE OF TRADE WILL BE PART OF THE CONTRACT BETWEEN US.

Price Quotes: Quoted pricing will be held until the specified expiry date. Prices are subject to change under exceptional circumstances including manufacturer price changes and foreign exchange fluctuations of more than 5%. Every effort will be made to honour pricing as quoted.

Installation & Training: Unless otherwise stated, all installation and training i s extra. Installation and training are only available on selected items and in selected locations. All costs are extra and must be quoted separately and in writing. Remote support is offered (by CSP or manufacturer) for all items, based on phone, fax or e-mail communications. If you have any questions, please contact the CSP sales person for your region.

Delivery times: Most items requiring calibration or configuration are built to order. This generally requires a delivery time of 3 to 5 weeks for North American orders. Some items, such as supplies are carried in inventory and can usually ship within 1-3 business days. The expected delivery date will be shown on the order confirmation form, usually sent within 1 business day from the time of the order. CSP is not responsible for production delays by the manufacturer or delays in shipping or border crossings. Every effort will be made to expedite the process. Express shipping is available at extra cost. You will be informed of delays (or early shipping) through the online order status by logging in to your account on our webstore.

Payment: Payment in advance: First time customers must pay in full in advance by credit card, check, bank draft or wire transfer. These orders will not be processed until payment is processed and approved. If payment is by Visa, Mastercard or AMEX, please add 3% to quoted prices.

Terms: Net 30 day payment terms are available to North American customers to enable ordering by purchase order payable through invoicing. Requires completion of a short, no-fee credit application. Credit card payment will not be accepted as payment for invoices. Extra fees may be incurred for late or rejected payments. All international orders are payment in advance.

Taxes: All taxes are based on the shipping address on the order. Canadian Customers: Customers in NB, NS, NF, ON, BC are charged HST unless a valid exemption certificate is provided. All other provinces are charged GST unless a valid exemption certificate is provided. USA: no taxes apply to customers in the US or outside North America. Import fees or duties specific to certain goods may apply, these charges are billed separately by carrier to ship to address on sales order.

Shipping and handling: All shipping and handling charges are extra unless specifically stated otherwise in writing. Charges: although CSP does not charge extra handling fees for North American customers, we recommend that you consolidate orders as much as possible to minimize processing time and shipping costs. International shipments may be subject to extra handling fees based on location. A $15 processing charge applies for orders not meeting our $100 minimum. FOB: Standard orders are shipped FOB London, ON. Other arrangements can be made, including insured shipments, for additional cost. Shipping Method: All shipments (except drop shipments) are carefully packaged and inspected at CSP Medical. Domestic shipments are delivered via ground courier (usually UPS, FedEx or Purolator). Heavy shipments and dangerous goods are shipped domestically by truck, internationally by the most economical courier or customer specified courier. Special Arrangements: Rush shipments and insurance are available at extra cost. Dangerous goods and heavy shipments may incur extra courier and/or crating fees.

Returns: Products purchased from CSP Medical can be returned up to 15 days from date of shipment (subject to condition 2 below). Products must be returned as shipped, in like new condition with original packaging materials, including manuals and accessories. All products sold through CSP Medical are sold to the specifications shown in the product description. CSP Medical is under no obligation to accept returns on products that meet the published specifications. No returns or credits are permitted on custom manufactured or configured products. Privacy policy: Any product that stores patient data and images must have this removed before returning to CSP Medical. Returns Process: All returns must be authorized in writing by CSP Medical. Call us for a Case Number and Return Authorization Form. This form must be included with the returned goods. Returns must be shipped prepaid by the customer to CSP Medical, FOB London, ON. A minimum 15% restocking fee (more for some manufacturers) will be charged for all returns unless covered under warranty. Credit will be issued once return is accepted by the manufacturer. If any of these requirements are not met, delays or extra costs could be incurred. CSP Medical does not take responsibility for these issues if caused by customer neglect, error or deviation from these policies.

Damaged Items: For goods received in a damaged condition by the courier or trucking company, please follow this procedure:

  1. Note the damage to the delivery driver and immediately report the the damage to the delivery company,
  2. Keep all original packaging materials for inspection and possible repackaging,
  3. Report the damage to CSP Medical in writing within 2 days of receipt and provide a copy of carrier's report of damage when available,
  4. Some shipments may have carrier insurance. Claims are contingent upon the above procedures being followed.

Warranty: All products sold through CSP Medical are inspected for quality control by the manufacturer and again for damage and basic functionality at CSP Medical. If a product should fail to function properly within the manufacturer's warranty period, please contact us at 519-4732277. The solution will depend upon the manufacturer's policies, the location of the customer, plus the size, weight and type of product.
